take it from someone who worked at a newspaper. you can edit a page, write the headline, print it out, get it proofed, close the page and send it on to the printer, and something happens in between those last two steps that causes that. i think it’s caused by designing the page on a PC and having the printer open it using a Mac.
sure, the readers don’t care WHY it happens, but i’d bet my balls it wasn’t editor error.
